How to prescribe JUVMO

As part of JUVMO Complete, JUVMO is available exclusively through our in-network Specialty Pharmacies: AcariaHealth, Accredo, and CVS Specialty. Prescriptions can only be submitted through the Parkinson's Complete Portal. There are 3 steps to getting your patients started on their prescribed JUVMO therapy:
One.

Step 1: Prepare

  • Determine whether to opt your patient into the JUVMO Free Trial Program†
  • Gather necessary clinical documentation
  • Navigate to the Parkinson's Complete Portal (ParkinsonsComplete.com) or use a faxable Prescription & Enrollment Form
Two.

Step 2: Prescribe

  • Prescribe JUVMO Titration Pack and 5 mg maintenance dose
  • Submit Prescription & Enrollment Form (after obtaining patient HIPAA consent)
  • Set patient expectations about JUVMO Complete outreach and Specialty Pharmacy
Three.

Step 3: Pursue Coverage

  • JUVMO Complete will help you and your office with navigating the JUVMO insurance landscape
  • Submit documentation for patients requiring a medical exception. (Templates are available on both this website and the Parkinson's Complete Portal)
  • Upon coverage determination, JUVMO will be dispensed by 1 of 3 Specialty Pharmacies: AcariaHealth, Accredo, or CVS Specialty

Benefits Investigation Identifies coverage status, pharmacy benefit, mandated Specialty Pharmacy, estimated patient OOP costs, and payer requirements for your patient
Pursuit of Coverage Provides information for prior authorizations (PAs) and medical exception requests. When a Letter of Medical Necessity (LMN) is needed, there are templates you can use to help draft the letter
Denial & Appeals Monitors the status of your prior authorization/medical exception request and provides information to help navigate the appeal process if a denial is received
Financial Assistance If an insurance delay or denial occurs, resources are available to help your patient adhere to their prescribed therapy, including our commercial Bridge Program and other possible options for eligible patients

For Commercially Insured Patients:

  • Savings Program: Once coverage is established, eligible patients may pay as little as $0/month.‡ Automatically applied at the pharmacy, no physical card required
  • Bridge Program: Eligible patients experiencing an insurance delay or denial can receive JUVMO at no cost for up to 24 months while coverage is being established

For Medicare Part D & Medicare Advantage Patients:

Help educate patients who may be eligible for government programs on how they can manage out-of-pocket costs, including:

  • Medicare Prescription Payment Plan (MPPP):  Government plan spreads out-of-pocket costs across monthly installments (i.e., Cap & Smooth)
  • Medicare "Extra Help" Federal Program: Helps cover Part D prescription costs for patients with limited income and resources
